The South East Grey Community Health Centre (SEGCHC) is a non-profit organization committed to providing primary care services and health programs to the residents of the Municipalities of Chatsworth, Grey Highlands, Southgate and West Grey. We are committed to building a healthier South East Grey community through: health promotion, illness prevention, and comprehensive, client-centered primary health care.
Position: Social Worker
Region: Grey and Bruce
Description: SEGCHC is currently seeking a full-time (37.5 hours per week) Social Worker to cover an 18 month contract, with possible extension. Reporting to the Clinical Director, the Social Worker will work as part of a dynamic, inter-professional team, providing patient advocacy, counselling/therapy and other therapeutic functions. The Social Worker will participate in the development, implementation, monitoring and evaluation of programs and services for individuals, families and communities.

Qualifications:
- Honours Bachelor or Bachelor of Social Work degree from a recognized University
- Current membership and in good standing with the OCSWSSW
- Demonstrated experience of case management, system navigation and advocacy
- Knowledge and proficiency in current, evidenced-based methods and practices of social work in primary care settings and community, with an emphasis on chronic disease management, health promotion and risk reduction
- Experience in mental health & addictions considered an asset
- Experience in an inter-professional setting will be considered an asset
- Experience with creating, implementing, facilitating and evaluating groups
- Understanding of social determinants of health and their impact on health and well-being
- Efficient computer skills, working knowledge of electronic medical record and documentation

Terms of Employment: Temporary, full-time (18 months)
