Service Description: Release planning case management to non-violent adults with a serious mental illness or serving sentences in the Stratford Jail, Elgin Middlesex Detention Centre, St Thomas Forensic Unit and Central North Correctional Centre
  • works with incarcerated individuals and their support person(s) to formulate a plan intended to increase the success of community reintegration
  • connection to resources and psychiatric services
  • urgent consultation to families, individuals, service providers, particularly police officers, when dealing with an individual who has, or maybe about to commit an offense

Eligibility / Target Population: Adults, youth and families who are affected by substance use and dependence, gambling/behavioural issues, and/or mental health concerns
