Service Description: Medical assistance in dying is available for people who have a serious and impossible to reverse medical condition * talk to doctor/nurse practitioner about options for treatment and care

Fees: None
Application: Must call the care coordination service to provide necessary information * a doctor or nurse practitioner will determine if you meet the eligibility requirements
Eligibility / Target Population: Must be 18 years of age or older * eligible for publicly funded health care services in Canada * be able to provide informed consent * have a grievous and irremediable medical condition
