Service Description: Medical assistance in dying is available for people residing in Canada who have a grievous and irremediable medical condition * drugs that end life are either directly administered by the physician or nurse practitioner or prescribed and then taken by the patient themselves

Patients should discuss with their physicians or nurse practitioner for information * physicians and nurse practitioners are the ones who provide medical assistance in dying services

Application: Informed consent must be given (includes knowledge of and understanding of medical diagnosis, available forms of treatment, available options, including palliative care), a written request must be submitted, medical assessments are required and a ten-day waiting period is required * additional provincial requirements may be necessary
Eligibility / Target Population: Must be:
  • eligible for publicly-funded health services in Canada
  • 18 years of age or older
  • mentally competent, including in health care decisions
  • diagnosed with a grievous and irremediable medical condition
  • voluntary requesting medical assistance in dying without outside pressure or influence
  • able to give informed consent
