Service Description: Provides on-site personal support services to eligible individuals

Helps with daily activities and supportive services to maintain independence in the community at designated buildings * a hub model providing support to the community as well as the designated buildings

Dressing
Mobility Assistive Devices such as mechanical lifts are the responsibility of the client
Personal hygiene
Monitoring medication Includes medication reminders, observation and assistance
Routine personal activities of living
 
Homemaking Services
Housekeeping Light housekeeping including bathroom cleaning, vacuuming, dusting, sweeping, mopping * no heavy housework such as outside windows, yard work or moving furniture
Shopping Workers will help clients prepare shopping lists and will also assist with grocery shopping where no home delivery is available
Banking, paying bills Workers may assist clients to prepare payment for bills and deliveries
Planning menus, preparing meals
 
Security Checks/Reassurance Services

Other
In home exercise program Workers will assist clients to follow physiotherapist prescribed exercises as well as basic daily regimes
Congregate Dining Available in some buildings
Care co-ordination is also provided * the Assisted Living Supervisor coordinates all elements of client care based on assessment results and conversation with the client and family * Supervisor connects with appropriate community resources such as the family primary health care professional, community support services CCAC, mental health services and other services as required
