Service Description: Inpatient clinical consultation, assessment, treatment, education, research and evaluation

Acute Care of the Elderly Unit - 14-bed acute care unit for patients requiring investigation and short term management of complex health issues

Consultation Liaison Team - Integrated medical and psychiatric consultation, liaison and ongoing follow-up for referred older adult inpatients in acute care settings

Geriatric Emergency Management - Program to keep older adult patients out of the hospital * advanced practice nurse provides assessment and clinical consultation for older people with complex health issues in the emergency room * GEM nurse also focuses on education with emergency room staff, patients and families

Geriatric Psychiatry Program - Program for older adult clients with late on-set mental illness or severe behavioural disturbances

Geriatric Rehabilitation Unit - 30-bed inpatient rehabilitation for clients with multi-factorial health problems (medical, functional, psychological, social) * length of stay approximately 4-6 weeks

Musculoskeletal Rehabilitation Unit - Inpatient program for those with complex musculoskeletal problems needing intensive and comprehensive rehabilitation services

Fees: Covered by OHIP * some transportation costs
Application: Referrals may be initiated by health care providers, older adults, or family members/informal caregivers * referrals by a doctor must be made through the centralized intake and triage * for rehabilitation programs medical referral required and access coordinated through the Home and Community Care Support Services South West Region
Eligibility / Target Population: Older adult aged 65 and older, and people with disabilities, who need physical, social, or emotional supports * for rehabilitation programs must meet Home and Community Care Support Services South West Region criteria
