Service Description: Health centre focused on family and community health * health professionals assess and treat non-life-threatening injuries or illnesses * referrals to other local health services and personal development groups * seasonal flu vaccine * walk-in counselling and RAAM (Rapid Access Addiction Medicine Clinic)

Community-led health teams develop programs to improve the social supports and conditions that affect long-term health
 
Counselling Services - Confidential counselling sessions for anyone who is dealing with stressful life changes, feelings of anxiety, depression, past trauma and other concerns
 
Dietitian Services - Nutrition services for all life stages * personal support in group and/or one-to-one sessions with a certified dietitian * programs include:
 
OHIP-Insured Physiotherapy Clinic - Community physiotherapy clinic * health professionals assess and treat conditions that affect the musculoskeletal, neurological or cardiovascular system following a recent injury, illness, accident, hospital stay, or a flare up or worsening of symptoms that has led a decline in function.
 
Outreach - Outreach staff assist with system navigation, community resources, transportation, advocacy and filling out forms 
 
Youth Outreach and Support - Our wraparound approach focuses on empowering you to identify and work toward goals in:
  • Education and skill building
  • Employment
  • Financial stability and literacy
  • Health and wellbeing
  • Identity, culture and belonging 
  • Recognizing your needs and advocating for them appropriately 
  • Completing paperwork and submitting applications
  • Creating your long-term support network 
  • Referrals to community agencies
  • Working through system challenges
