Service Description:

Diabetes education and support for those without a family doctor or who live with various challenges that make managing diabetes more difficult
Multidisciplinary team assists patients to become diabetes self-managers * helps patients access the resources they need to manage their diabetes * team also assists family doctors in London and area to build capacity for diabetes care in their own practices * team includes three doctors, a nurse practitioner, certified diabetes educators (nurses and dietitians), and a social worker
