Service Description:

Assisted Living - Scheduled and unscheduled support for high-risk older adults or adults with physical disabilities * available in individual homes, group homes or apartment clusters

Attendant Outreach - Scheduled support to assist adults with physical disabilities with daily activities to live independently at home

Fees:

Support services are free * rent geared to income

Application:

Call * referral through the Home and Community Care Support Services South West Region * to register for Huron or Perth programs, call Huron Perth Community Support Servivces at 1-844-482-7800
