Service Description: Help to buy, repair and maintain a wide variety of mobility equipment or devices that increase functional independence

Some devices that may qualify for funding include:
  • manual and power wheelchairs
  • walkers
  • floor patient lifts
  • commodes, tub transfer benches and raised toilet seats
  • transfer aids
  • scooters
  • replacement batteries
  • Knee Ankle Foot Orthosis (KAFO)
  • Ankle Foot Orthosis (AFO)
  • repairs for existing devices

Application: No referral required * complete application form and submit by mail, fax or email
Eligibility / Target Population: People 19 years and older with a physical disability and in financial need that require the use of an assistance device
