Service Description: Supported living option includes assistance with daily living skills, such as personal care, meal preparation, medication monitoring, budgets, house management, and community involvement

Older adults are provided with minimal to moderate assistance for tasks such as homemaking or personal care

Scheduled and unscheduled visits available to support personal independence

Lifeline - Available in-home and in 3 designated buildings
