Service Description: Residential program for people with a serious mental illness who may also have physical health concerns and need access to 24-hour supports with clinical interventions for an extended period of time

Opportunity for persons who have been hospitalized in a specialized psychiatric facility (divestment), or who are at risk of hospitalization, to move toward reintegration in the community

24-hour specialized program of psychosocial rehabilitation treatment and clinical support in a home-like environment and interactive community setting * supports provided by a multidisciplinary team including nursing staff and mental health and addictions workers

Single location that provides an alternative to hospital or facilitates an early discharge from hospital

Fees: Subsidized monthly rent

Eligibility / Target Population: Adults and youth 16 years and older with persistent mental illness who are characterized as complex with a history of lengthy and/or multiple hospital stays * have rehabilitation potential and readiness as well as a desire for improved mental health
