Thames Valley Family Health Team Secures Funding for Innovative Health and Homelessness Primary Care Response Plan

Thames Valley Family Health Team (TVFHT) is pleased to announce the acquisition of substantial funding from the Ontario Government to launch a pioneering Health and Homelessness Primary Care Response Plan in collaboration with local healthcare partners. This initiative represents a significant step towards addressing the critical intersection of health and homelessness within our community.

Aligned with the emerging Health & Homelessness (HH) Whole of Community System Response from the City of London, the Primary Care Services facilitated by this initiative will provide comprehensive support to those experiencing homelessness or at risk of homelessness.

Key features of the Health and Homelessness Primary Care Response Plan include:
  • Onsite comprehensive primary care: nurse practitioners (NPs) will deliver comprehensive primary care services directly at a designated Community Hub location. This approach ensures accessibility and continuity of care for individuals in need. Recognizing the importance of meeting patients where they are, TVFHT NPs will foster stronger patient attachments to primary care and facilitate ongoing support.
  • Interprofessional team support: Interprofessional team members will be readily available for consultations and appointments, ensuring a holistic approach to healthcare delivery.
  • Integration with community resources: The primary care model will seamlessly integrate with the broader resources of TVFHT and its community partners, ensuring that patients are helped to find the services they need.
